Hello, UPN Veterans Jakarta Big Family and the General Public!

UPN Veteran Jakarta is holding an open vaccination program for the entire UPNVJ extended family and the general public in order to increase herd immunity. The types of vaccines that will be administered are the ASTRA ZENECA and SINOVAC (CORONAVAC) vaccines with a total quota of 1,000 doses.

For the implementation of the vaccination program, it will be carried out on:

🔴 Time: 07.30 to 12.00 WIB
🔴Place: UPN Veteran Jakarta, Pondok Labu Campus

‼️ONLINE PARTICIPANT REGISTRATION FLOW‼️
1. Participants register via google form (bit.ly/VaksinasiUPNVJHUTRI76)
2. Attendees arrive 30 minutes before the chosen vaccination time to take a queue number and a printed form
3. Participants are required to re-register and must comply with the time session for the vaccination options THAT HAVE BEEN SELECTED on the google form.
4. The committee will provide a queue number if the participant is confirmed in the vaccination participant data
5. Participants who have been given a queue number will wait for their turn to be screened, followed by vaccination

‼️TERMS‼️
1. Participants register themselves via the Google form (bit.ly/VaksinasiUPNVJHUTRI76)
2. Vaccination participants are required to bring a photocopy of their KTP or photocopy of KK
3. Participants of the first dose of vaccine aged over 18 YEARS will receive ASTRAZENECA Vaccine and participants of the first dose of vaccination aged 12-17 YEARS will receive SINOVAC Vaccine (CORONAVAC).
4. Vaccination participants must bring personal stationery.
5. Participants are advised to have breakfast first and not vaccinate on an empty stomach.
6. Participants must comply with health protocols (wearing masks, maintaining distance, washing hands, staying away from crowds, and limiting mobility).
7. The vaccine doses given are Astra Zeneca and SINOVAC with a total quota of 1,000 doses.
